New Patients
Completing these forms prior to your visit will shorten your registration time.
Allergy testing may be done during your visit and can take anywhere from 1 to 2 hours. If this appointment is not for a child, we strongly suggest that children not accompany you on your visit. Allergy testing requires the patient to be perfectly still, and we have found that patients with children are unable to do so.
DO NOT TAKE ANTIHISTAMINES FOR 7 DAYS PRIOR TO YOUR VISIT. Asthma medication, as well as other medications, may be continued. Please bring all medications you are currently taking (or a complete list) to your appointment.

We accept all major insurance plans. Please contact us if you have a plan that is not on this list to confirm participation.

Policies
- Due to food allergies, no food is permitted in our offices.
- No service animals are permitted in our offices.
- Please be mindful of our patients’ sensitivities and help us create a fragrance-free environment by refraining from using perfumes and other scented items prior to visiting our offices.
- No concealed weapons are allowed on our property.
- Food desensitization appointments missed without a 24-hour prior notification are assessed a $250 fee.
- All other appointments missed without a 24-hour prior notification are assessed a $25 fee.
- Allergy serum transfers are assessed a $25 fee if using LSO or a $40 fee if using FedEx.
- Records transfers are assessed a $25 fee.
- NSF checks are assessed a $35 fee.
